Skilled Nurse (RN/LPN)
- Perform clinical assessments and develop individualized care plans.
- Administer medications, wound care, and specialized medical treatments.
- Coordinate with physicians, caregivers, and family members to ensure continuity of care.
Special Skills: Strong clinical judgment, proficiency with home-based medical technology, and exceptional patient advocacy. A current, valid nursing license for the state of Ohio is required.
Physical Therapist (PT)
- Evaluate patient mobility and design customized rehabilitative programs.
- Guide patients through exercises to improve strength, balance, and range of motion.
- Educate caregivers on safety techniques, equipment use, and home-based support strategies.
Special Skills: Deep knowledge of geriatric rehabilitation, manual therapy techniques, and home-safety auditing. A current, valid Physical Therapy license for the state of Ohio is required. Please note that our agency also hires for Physical Therapist Assistant (PTA) roles.
Occupational Therapist (OT)
- Help patients engage in daily tasks through therapeutic, goal-aligned activities.
- Recommend adaptive equipment and home modifications to improve safety and ease.
- Design sensory or cognitive intervention strategies to support neurological recovery.
Special Skills: Expertise in adaptive equipment, cognitive assessment, and ergonomic home-environment design. A current, valid Occupational Therapy license for the state of Ohio is required. Please note that our agency also hires for Occupational Therapist Assistant (OTA) roles.
Home Health Aide (HHA)
- Assist patients with daily living activities such as bathing, dressing, and grooming.
- Monitor vital signs and report changes in physical or mental status.
- Provide companionship and light housekeeping to maintain a safe, comfortable home environment.
Special Skills: A calm, compassionate presence; patience and understanding when supporting seniors and adults with disabilities; and the physical ability to assist safely with mobility and transfers.
Deaf Services Specialist
- Provide direct care and support for Deaf, Deaf-Blind, or hard-of-hearing clients.
- Ensure clear communication using ASL or the client's preferred communication methods.
- Assist with access to community resources, visual supports, and adaptive technology.
Special Skills: Fluency in American Sign Language (ASL), strong cultural competency within the Deaf community; and familiarity with visual communication tools, assistive devices, and adaptive technology that support communication access.
