The Heart of Whole Person Care: Your Family Intake Guide
Understanding your loved one goes far beyond medical notes or diagnoses. Their routines, joys, history, and values shape how we support them each day. This packet helps us learn the full picture — so we can care for them in a way that feels familiar, comfortable, and truly personal. It’s the first step in getting to know your family and building a care plan that feels right.

Where You Start:

Step 1: Become the Historian
Sharing your loved one’s story helps us understand who they are beyond their care needs. Tell us about their background, traditions, proud moments, and the experiences that shaped their life. These details help us show up with familiarity and respect.
This is the heart of our intake process — learning who they are before we learn what they need.

Step 2: Detail Daily Rhythms
Everyone has a natural flow to their day. Let us know their morning habits, favorite foods, music they enjoy, or small comforts that make them feel at ease. These rhythms guide both intake and the early stages of care planning, helping us create support that feels seamless.

Step 3: Define Communication
Help us understand how your loved one connects best — whether through gestures, certain phrases, ASL, or simply knowing which topics bring joy. Your insight ensures every interaction feels comfortable, clear, and meaningful - and helps our caregivers communicate with confidence from the very first visit.
Where We Start:
01
Initial Contact
Reach out to our team using the form below. A care coordinator will connect with you, answer your questions, learn about your loved one’s needs, and help you feel steady and supported as we move into next steps.
02
Clinical Assessment
A registered nurse visits your home to complete a gentle, thorough assessment. We look at health needs, safety, comfort, and daily routines — all the pieces that help us build a plan that truly fits your life.
03
Care Begins
Once your physician signs the plan of care, your care team begins visits. We show up with consistency, compassion, and the clinical support needed to help you or your loved one feel secure at home.
